Managing Sleep Apnea with MAD Appliances

Sleep apnea is a serious sleep disorder characterized by repeated interruptions in breathing during sleep. These interruptions, or apneas, can lead to reduced oxygen levels in the blood, fragmented sleep, and a host of associated health problems. Effective management of sleep apnea is crucial for improving quality of life and overall health. One of the most effective treatments for sleep apnea is the use of a Mandibular Advancement Device (MAD). This article explains how we use MAD appliances, along with advanced diagnostic tools like rhinometry and pharyngometry, to tailor a personalized treatment plan for our patients.

What is a MAD Appliance and How Does It Work?

A Mandibular Advancement Device (MAD) is a custom-made oral appliance designed to treat obstructive sleep apnea (OSA). It works by gently repositioning the lower jaw (mandible) forward, which helps to keep the airway open during sleep. The device is fitted to fit comfortably in your mouth and is adjusted to move the jaw into a position that prevents the collapse of soft tissues at the back of the throat, which can obstruct the airway. By maintaining an open airway, the MAD appliance reduces or eliminates the apneas and hypopneas that disrupt your sleep. This non-invasive alternative to Continuous Positive Airway Pressure (CPAP) therapy is particularly beneficial for patients with mild to moderate sleep apnea and offers a comfortable and effective solution for improving sleep quality.

Step 1: Comprehensive Sleep Testing

Before initiating any treatment for sleep apnea, a physician must diagnose the condition. We work with a third-party company that connects you with a physician via telemedicine. This company facilitates an initial consultation, orders and mails a take-home sleep test, and sets up a telemedicine appointment for you to review the findings with a qualified doctor. Based on the results and the doctor’s recommendations, if a MAD appliance is deemed appropriate, you will be referred back to our office with a prescription for the device.

Step 2: Records Collection and Diagnostic Tools

To create a custom MAD appliance, we need to gather specific records and perform detailed diagnostics. This includes:

  • Pharyngometry and Rhinometry: These diagnostic tools are crucial for ensuring the MAD appliance is positioned optimally.

Pharyngometry measures the dimensions of the pharyngeal airway, allowing us to precisely determine the optimal jaw position to maximize airway patency. Without pharyngometry, the optimal jaw position would be determined through guesswork, which may not be as effective. Rhinometry measures nasal airway resistance, helping us assess nasal airflow and patency, which is essential for complementing the MAD’s effectiveness.

  • Digital Impressions: Accurate digital impressions of your teeth and gums are taken to ensure a precise fit for the MAD appliance.
  • CBCT Scan: A Cone Beam Computed Tomography (CBCT) scan provides detailed 3D images of your jaw, teeth, and airway, which are essential for precise appliance design.
  • Step 3: Delivering the MAD ApplianceOnce all records are collected and diagnostics are complete, we proceed to deliver the custom MAD appliance. The appliance is carefully fitted to ensure comfort and effectiveness in keeping the airway open. We will provide detailed instructions on how to use and care for the device.
  • Step 4: Follow-Up Sleep TestAfter you start using the MAD appliance, a follow-up sleep test is essential to confirm that the jaw is in the optimal position. This test helps us evaluate the effectiveness of the appliance and determine if further adjustments are necessary. Based on the results, we may adjust the appliance by advancing it slightly forward if needed. To ensure ongoing effectiveness, it is recommended to have a sleep study done once per year to monitor the appliance’s performance and make any necessary adjustments.
