Our Perio3 Protocol

Understanding Periodontal Disease

Periodontal disease is a multifactorial condition involving bacterial infections, genetic predispositions, systemic conditions, and environmental influences. Our approach targets these root causes through a combination of advanced treatments and personalized care, ensuring effective management and improved outcomes.

Our Perio3 protocol is designed for patients seeking an alternative to flap surgery, those who have been told their teeth are hopeless due to periodontal disease, or individuals who are faced with needing another round of SRP or “deep cleaning”. Unlike traditional methods, Perio3 focuses on addressing the root cause of periodontal disease, providing a treatment that not only removes buildup but promotes bone regeneration by supporting the bodies host response, resulting in long-lasting results.

The Limitations of Traditional Scaling and Root Planing/Deep Cleanings

Traditional scaling and root planing (SRP) is a common procedure for treating periodontal disease, but it has its limitations, particularly because it is a procedure that is performed “blindly.” In this context, “blind” SRP refers to the traditional method where the dentist or hygienist relies on tactile feedback and experience to clean beneath the gum line. However, without direct visualization, it’s challenging to ensure that all microscopic plaque and tartar are thoroughly removed. Studies show that up to 70% of build up under the gums can be left behind post treatment! This leads to incomplete cleaning, leaving behind bacteria and deposits that will continue to cause inflammation and damage to the gums and supporting bone. In addition to being a “blind” procedure, often times the cause of periodontal disease and associated risk factors are never addressed with traditional methods, making them less effective at treating periodontal disease.

A Note on Flap Surgery/ Periodontal Surgery

Flap surgery is a traditional treatment for advanced periodontal disease, where the gums are surgically lifted to allow deep cleaning of the root surfaces and removal of tartar buildup. However, even with this invasive approach, studies show that up to 24% of buildup can still be left behind. The procedure is not only expensive but also comes with a challenging post-operative recovery, often involving discomfort and swelling. Additionally, patients almost always experience gum recession around the treated teeth, which can lead to increased sensitivity and aesthetic concerns.

Our Perio3 Protocol

We have combined Regenerative Periodontal Endoscopy (RPE), a cutting-edge procedure designed to treat advanced periodontal disease with precision and regenerative techniques with biological practices like ozone therapy and other topically applied therapeutic agents promoting healthier gums and supporting long-term oral health. We focus heavily on weighing your specific risk factors and coming up with an optimized and custom plan to address the root cause of your periodontal disease.

Comprehensive Risk Assessment

To tailor your treatment, we evaluate individual risk factors through:

  • Salivary Diagnostic Testing: Identifies pathogenic bacteria.
  • Blood Work: Assesses systemic deficiencies.
  • Genetic Testing: Determines if you are a “hyper-responder” with an overactive immune system.
  • Lifestyle Screening: Assesses habits, diet, and other lifestyle factors that may influence your periodontal health.

The Role of Periostat

Periostat is integral to our treatment plan, offering unique benefits for managing periodontal disease without functioning as a typical antibiotic:

  1. Targeted Action: Periostat contains doxycycline, which inhibits matrix metalloproteinases (MMPs) responsible for tissue destruction, protecting the structures that support your teeth.
  2. Reduces Inflammation and Bleeding for Optimal Visualization During Procedures: Periostat helps manage inflammation and reduce bleeding, improving the visibility and effectiveness of treatments.
  3. Supports Healing: By controlling inflammation, Periostat enhances the effectiveness of procedures like RPE and Emdogain, contributing to better overall outcomes.

Importantly, studies show that Periostat does not impact the gut microbiome, making it safe for long-term use without the concerns associated with traditional antibiotics.

Microscopic Removal of Build up using Endoscope

Our Endoscope is a small 20X camera that goes under the gums and allows us to see and remove build up that remains under the gums. It does not require any cutting of the gum tissue and allows your practitioner to be very precise and minimally invasive, leading to better healing and fewer post-operative complications. Unlike surgery, perioscopy does not increase inflammation or cause additional gum recession after treatment.

Application of Emdogain

Emdogain is a protein matrix gel that is placed between the gums and tooth once they are clean. It plays a crucial role in our approach by promoting the regeneration of healthy periodontal tissues and supporting healing.

Ozone Injections: Enhancing Healing and Lymphatic Drainage

As part of our comprehensive periodontal protocol, we incorporate ozone injections to further support healing and recovery. Ozone injections involve administering ozone gas into periodontal tissues, where it promotes healing and supports proper lymphatic drainage. The antimicrobial properties of ozone help eliminate bacteria, enhancing tissue repair and reducing inflammation. Additionally, ozone injections improve lymphatic flow, which supports the body’s natural healing processes, and accelerates recovery, helping to reduce recovery time and contribute to faster and more effective periodontal treatment.

Perioprotect Trays: An Additional Tool

Perioprotect trays may be recommended as part of your treatment to further support your periodontal health. These custom trays are designed to deliver therapeutic agents directly to periodontal tissues, providing targeted delivery for localized treatment of affected areas. The trays offer antimicrobial support that complements the effects of other treatments enhancing the overall effectiveness of your periodontal care. Additionally, Perioprotect trays are convenient and easy to use at home, making it easier for you to comply with your treatment plan and achieve optimal gum health. By integrating these trays with other treatment modalities, we aim to maximize your results and ensure long-term success in managing periodontal disease.
